We accept all major insurances including Aetna, Bluecross blue shield, United HealthCare, Tricare, Humana Military, Molina, Ameri Health, Cigna, Medicaid, Medicare, Carelinx, Kaiser Permanente, Anthem; and all major credit/debits cards for self-paying clients. Always check with insurance carrier to determine benefits and network coverage.
Payment for service rendered – including self-pays/insurances are due 24 hours PRIOR to the appointment.
